Welcome to Fairview Dental! To help us get you into the chair faster, please complete your new patient paperwork before your appointment. Choose the option that works best for you:
š New Patient Registration Form
Option 1: Fill out online & email (Recommended) Click the link below to open the fillable PDF, type your information directly into the form, save it, and email it back to us at drl@brentwoodfairviewdental.comĀ before your visit.
- Option 2: Print & bring with you Download and print the PDF, fill it out by hand, and bring the completed paperwork with you to your appointment.
- Option 3: Fill it out in person If you prefer to fill out your paperwork at our office, no problem! Please arrive 10 to 15 minutes before your scheduled appointment time so you have plenty of time to complete it in our reception area.
š” Friendly Reminder: If you choose Option 2 or 3, please remember to bring your dental insurance card and a photo ID so we can verify your benefits!
What to Bring to Your Appointment
In addition to your completed New Patient Form, please bring:
- A valid photo ID
- Your insurance card (if applicable)
- A list of current medications
- Any recent dental X-rays or records from previous dental providers
